Obtaining Your Images and Reports
Patients, or their authorized representatives, may obtain free copies of their images and reports from Advanced Radiology in their choice of format:
Patient Portal
View images and read reports directly from MyInfo.AdRad.com, our secure, online Patient Portal. All Advanced Radiology patients may access their images and reports immediately upon submission of the radiologist’s final signed dictation.You must create an account to gain access to our password-protected Portal. We strongly suggest that you allow your referring physician time to review your images and reports before discussing it with them.
Encrypted Email
Secure email, with 256-bit encryption and virus scan. Complete an Information Release Authorization form (available at our clinical sites or by downloading the PDF below) and either submit it in person or fax it to Medical Records at 203.380.3289. Allow one working day for processing of your request. Please note: information you recieve via email cannot be forwarded to or shared with a third party.
You may purchase your images on CD or USB
Either of which may be picked up in person at our Shelton business office, or delivered via your choice of courier (delivery charges will be added to the total cost of the information).
Compact Disc or USB Flash Drive. Images can be stored on CD or USB.
Patients must first complete an Information Release Authorization form (available at our clinical sites or by downloading the PDF below) and either submit it in person or fax it to Medical records at 203.380.3289.
Records may be delivered (at additional cost) or picked up at our Shelton Business Office:

For in-person pickup, please allow 24-48 hours. Please see below for fees associated with each format and delivery method.
CD: $10.00 (additional delivery fees may apply)
When a release is submitted and received an Advanced Radiology Representative will contact you for your payment in advanced.
